Is the WebLogic Server included in the price of the licensing?

You will not need any additional hardware to run AuraPlayer. You can run AuraPlayer either on the existing EBS WebLogic server or on a new Tomcat server which is a freeware. So you should not need to pay any additional licenses.  

Is only 1 WebLogic server required or do we need a WebLogic server for each of our test instances as well?

We only need 1 AuraPlayer server install for you to be able to run services for all your test environments. However you will need to have a license for each URL that you want to record or play a service on. Meaning if you would like to record on 3 environments you will need 3 AuraPlayer server licenses although you can have the AuraPlayer technology installed on only 1 server.

Do you know of a way to run the Oracle forms backend component outside of weblogic... For example Tomcat?
 
Sorry to say, but Oracle Forms is unable to run on any backend middleware except Weblogic. That said we have different options:
1) Run weblogic in the cloud on the IAAS cloud service.
2) Run the back-end weblogic on premise (even 10g) with AuraPlayer running ( on Tomcat or in a IAAS cloud server ) and have NON a java applet - HTML based UI in the cloud with a modernized front-end using Oracle's next generation development tools such as Visual builder cloud service.
 

Does the Standard edition of the Weblogic server enough to run AuraPlayer? or it needs enterprise edition? 

Yes. Standard edition can run the AuraPlayer server. And if you want to save money, you can even run AuraPlayer using Tomcat which is 100% free.
